All providers expire 12/31 of the year issued. Renewals are due by October 1 of each year. Form GID110 is used to renew both the provider and instructor – no renewal form is sent out by the Department. The fee for renewal is $50 for provider, $10 for each instructor. Provider renewals received after October 1 will be subject to an additional late fee of $15 for each instructor and $15 for the provider.
